1) Submit the Prospective Student form

All new AND returning students are required to submit the Prospective Student Form each year. After the March new parent/student meeting, once a student has decided to commit to taking band or guard as an elective the following school year, it is essential to complete the form below. We must have up to date information for every student on file.

IF A NEW PARENT/STUDENT MISSED THE MARCH MEETING BUT THE STUDENT STILL INTENDS ON TAKE BAND OR GUARD AS AN ELECTIVE NEXT SCHOOL YEAR, PLEASE COMPLETE THE FORM NOW! – THE SOONER WE BUILD YOUR RECORD, THE SOONER WE CAN KEEP YOU INFORMED!

2) Submit Conroe ISD Physical form

DO NOT COMPLETE THIS SECTION UNTIL IT IS UPDATED WITH 2021 INFORMATION!!

Conroe ISD and the UIL have modified the physical requirements for 2020-2021.

Incoming 7th graders, new students to CISD, and new participants for Athletics, Color Guard, Marching Band, Cheerleading, Drill Team, and ROTC will need to get a physical for 2020-2021.

Unfortunately, this year, it has not been possible for us to coordinate an on-site opportunity to do this, so please plan accordingly.

(If a student has a current and completed physical for the 2019-2020 year on file, parents have the option to use their child’s physical for the upcoming 2020-2021 year).

A CISD Pre-Participation Physical form MUST be submitted prior to the first day of summer camp. Click on the button below for complete instructions and Instructional video.

Everyone must complete the 2020-2021 Pre-Participation Form and Medical History Form online using the “Save & Submit” button. If you choose to use your child’s 2019-2020 physical, please note that family history or other medical conditions may warrant the training staff to request further medical documentation and/or an updated physical.

All paperwork must be submitted online through SportsWare. This will help expedite the submission process and improve the College Park Sports Medicine Team’s communication ability regarding all paperwork. After completing the CISD Pre- Participation Packet and Medical History Questions online, parents will be able to upload doctor notes regarding restrictions, and clearances for the Sports Medicine Team to review. When you submit medical documentation digitally it MUST be in a scanned format (PDF) not a picture of the document (JPEG). If you have created a SportsWare account or updated your profile prior to May 18th, 2020, you will have to update the profile again.

2021 information

Parents can set up SportsWare accounts at anytime, from 7th-12th grade. The CP Sports medicine website contains and instructional video and lots of advice on how to submit correctly, but the most common issues encountered are as follows:

Paperwork digitally signed in student’s name (must be in parent’s)
Both online documents not completed using “save & submit” (uploading similar documents does not mean it doesn’t need digital submission)
Uploading scanned documents that are not legible
Not explaining “yes” answers on the medical history
